Thực trạng vấn đề cần cải tiến:

Khoa Sản bệnh là khoa lâm sàng tiếp nhận điều trị và chăm sóc các sản phụ có thai kèm bệnh lý. Trong đó bệnh lý đái tháo đường thai kỳ nếu không được kiểm soát tốt ảnh hưởng rất lớn đến thai phụ và trẻ sơ sinh như sẩy thai, thai lưu, sinh non, tăng huyết áp trong thai kỳ, đa ối… Nên việc theo dõi sát chỉ số đường huyết tại khoa sản bệnh là vô cùng cần thiết. Hiện tại khoa Sản Bệnh chưa có phiếu theo dõi chỉ số đường huyết tại khoa

Xuất phát từ thực tế trên khoa sản bệnh xây dựng phiếu theo dõi chỉ số đường huyết với mục tiêu

1. Xây dựng được phiếu theo dõi chỉ số đường huyết đầu tiên tại khoa Sản Bệnh.

2. Tăng tỷ lệ hài lòng của điều dưỡng trong chăm sóc người bệnh trước và sau khi sử dụng phiếu.

Phương pháp nghiên cứu:

1. Dựa vào 5S khoa để giải quyết vấn đề: Sàng lọc, sắp xếp, sạch sẽ, săn sóc, sẵn sàng

2. Đánh giá sự hài lòng theo thang đo Likert 5 mức độ

Kết Quả

Sau cải tiến, hầu hết các điều dưỡng điều hài lòng với phiếu theo dõi đường huyết mới, tỷ lệ dao động từ 94,4%-100%. 100% điều dưỡng nhận định phiếu theo dõi chỉ số đường huyết mới thuận lợi trong việc ghi chép trên phiếu và giúp điều dưỡng dễ đối chiếu y lệnh thuốc và đường huyết mục tiêu.

The Department of High risk pregnancy is a clinical department that receives treatment and care for pregnant women with pathologies. In which gestational diabetes, if not well controlled, greatly affects pregnant women and newborns such as miscarriage, stillbirth, premature birth, hypertension in pregnancy, polyhydramnios. Therefore, close monitoring of blood sugar in the obstetrics and gynecology department is extremely necessary. Currently, the High risk pregnancy Department does not have a blood glucose monitoring sheet at the department.

Derived from the reality in the high risk department, the glycemic index monitoring sheet was built with the goal of

1. Developed the first blood glucose monitoring sheet at the high risk Department.

2. Increase the satisfaction rate of nurses in patient care before and after using the voucher.

Research Methods:

1. Rely on 5S faculty to solve problems: Sort, arrange, clean, take care, ready

2. Rate the satisfaction according to the 5-point Likert scale

Result

After the improvement, most of the nurses are satisfied with the new blood glucose monitoring sheet, the rate ranges from 94.4%-100%. 100% of nurses said that the new glycemic index monitoring sheet is convenient for recording on the sheet and makes it easier for nurses to compare medication orders and target blood sugar.

The department met the expected target and built the first blood glucose monitoring sheet in the Department of high risk pregnancy.
